Our company reserves the right to adjust our fee schedule at any time when deemed necessary.

An additional 15% late fee will be added for each day an invoice is not paid as agreed.

Nursing opinions contained in each report completed are given to a reasonable degree of nursing certainty, based on the documents furnished to me by each attorney client and or legal representative.

I respectfully reserve the right to modify or add to each opinion upon the furnishing of additional documents and/or relevant information.
